This volunteer application is used for all ministry areas, including Prison Ministry, Prevention Ministry, InZone, Jabbin’ for Jesus, Christmas Angel, and other ministry opportunities. As you complete the form, you will be able to select your ministry of choice and the type of service you are interested in.

Transform Ministries is a non-denominational Christian organization dedicated to creating a safe, nurturing, and faith-centered environment for children, youth, adults, and families. Because we serve vulnerable individuals and families, we take safety seriously.

As part of our commitment to safeguarding every person in our care, all volunteers are required to complete a background check. By signing your name on this application, you are giving Transform Ministries permission to conduct a background check and affirming our shared responsibility to create a secure, positive, and Christ-centered experience for those we serve.
